It only takes three easy steps to work at the MODA Center

Step 1 - Obtain a Food Handler Permit (required by ALL)

Step 2 - Obtain a OLCC Permit (required if 18 yrs or older)

Step 3 - Complete a Levy Background Check Form (required by ALL)

Step 4 - Complete Levy Restaurant Server Form (required if 18 yrs or older)
